  REGISTRATION FORM

   R     Please mark the classes you wish to attend, detach and return with your check.
R       Pre-payment of class fee is mandatory in order to be admitted to class. Phone, fax or E-Mail reservations are not
            accepted unless immediately followed by payment.
R       Class fees are not refundable and in case of class cancellation they will be applied to another class.
R       Requests for class credits must be made no less than 7 days before the class or the registration fee is forfeited.
R       Classes are subject to change. Students will be informed of changes as needed.
R       Alcoholic beverages are not included in class cost.  Please bring your own.
R       Gift Certificates are not refundable and have NO CASH VALUE.
